Can I Get Pregnant During Perimenopause?

As women transition out of their reproductive years, the ovaries don’t stop producing eggs immediately. Perimenopause is most often a gradual process that can last up to eight years, and during this time it’s still possible to get pregnant.

While the chances of conceiving naturally at age 45 and older – the typical age range for perimenopause – remain lower than in your 20s or 30s, pregnancy is still possible. This means you should continue using contraception if you’re sexually active and wish to avoid pregnancy.

The OB-GYN practice of Alan B. Patterson, MD, in Coral Springs, Florida, provides exceptional care to women through all phases of life. We counsel perimenopausal women on contraceptive options, and provide custom hormone replacement therapy and other symptom management solutions. Here’s what to know about the chances of getting pregnant during perimenopause.

Perimenopause and pregnancy

Perimenopause is the transition period where your ovarian function declines, paving the way for the end of your reproductive years. Many women are surprised to learn that they can indeed get pregnant during perimenopause.

During perimenopause, your ovaries produce fewer eggs and hormone levels fluctuate, often causing irregular menstrual cycles. However, ovulation doesn’t stop abruptly. Even if your periods are unpredictable or less frequent, you may still release an egg and become pregnant if you have unprotected intercourse.

Because it’s difficult to predict when ovulation will occur, pregnancy can happen unexpectedly. In general, contraception is recommended until you’ve gone 12 consecutive months without a menstrual period, which officially marks menopause.

Avoiding pregnancy during perimenopause

Your chances of getting pregnant naturally after the age 40 are around 10%. If pregnancy isn’t part of your plans, several safe and effective contraceptive options are available during perimenopause. These include:

  • Intrauterine devices (IUDs)
  • Contraceptive implants
  • Birth control injections
  • Barrier methods
  • Permanent sterilization
  • Progestin only birth control pills
  • Combination  birth control pills containing 10 mcg of estrogen, but no more than that

For many healthy, non-smoking women, low-dose combined birth control provides an excellent option during perimenopause. Besides preventing pregnancy, this option can help regulate irregular periods, reduce heavy menstrual bleeding, decrease hot flashes, and improve other common perimenopausal symptoms.

Dr. Patterson will recommend the contraceptive method that’s most appropriate for you based on your health history, symptoms, and future family plans.

If you’re hoping to get pregnant

Some women enter perimenopause believing they’ve missed their opportunity to have a child. While fertility naturally declines with age because egg quantity and egg quality decrease, natural conception is still possible during perimenopause.

The chances of becoming pregnant each month are lower than they were in earlier years, and the risks of miscarriage and chromosomal abnormalities also increase with age. However, many women in their 40s successfully conceive, either naturally or with fertility treatment.

If you’ve been trying to become pregnant without success, don’t assume it’s too late. A fertility evaluation can provide valuable information about your ovarian function, hormone levels, ovulation, and overall reproductive health.

Depending on your individual situation, Dr. Patterson may recommend lifestyle changes, medications to stimulate ovulation, intrauterine insemination (IUI), or in vitro fertilization (IVF). Every treatment plan is personalized to maximize your chances of achieving a healthy pregnancy.
